For groups of 2 or more enrolling and employer-contributory. AD&D option includes seat belt and air bag benefits. This plan also includes optional supplemental/voluntary life and increased coverage options for employee and dependents.

Guarantee issue amounts are based on total group volume. 2-year rate guarantee and discounts available for preferred industries.
