BEST Life Access Vision (Indemnity)

1. For groups with 2 or more when bundled with a BEST Life product
2. Available as stand alone for groups with 5 or more employees enrolled
3. Employer-contributory and voluntary available
4. No ineligible industries, industry loads or waiting periods
5. Access care from any licensed ophthalmologist or optometrist
6. Contacts in lieu of OR in addition to frames and lenses
7. Medically necessary contact lenses and Lenticular lenses included as eligible expenses
8. Flexible frequency, allowance and deductible options
9. EyeMed Vision Discount plan for Access Vision plan members, includes discounts for exams, glasses, lens options and add-ons, contact lenses and even laser vision correction are included with no limitations on frequency. Members can obtain these discounts through EyeMed's network.

BEST Life Vision PPO Plan

1. Network access to over 21,000 locations nationally
2. Contact lenses in lieu of eyeglass lenses
3. Frames allowance available in addition to contact lenses
4. Access to care includes optometrists, ophthalmologists, opticians, private practices and many leading optical retailers including LensCrafters, Target Optical, Sears Optical, JC Penny Optical and most Pearle Vision locations
5. 15% off retail price for Laser Vision Correction included
6. Materials Only plans available upon request

Laser Eye Surgery Discounts
