Caremark Prescription Drug Benefits
BEST Life medical plans include CareMark Prescription benefits.  CareMark’s network has over 60,000 participating pharmacies nationwide and offers the option to refill prescriptions by mail.

Plan participants are encouraged to use the Caremark Web site, www.caremark.com, to view the most current version of the drug list, as well as to review their prescription drug benefit information, request mail service orders, and research drug information.

HDHP Prescription Coverage
The Health Solutions HDHP plan provides a Caremark Discount Only card, and allows members to apply their prescription costs to the plan’s deductible and then coinsurance.

To receive a discount, members present their Caremark Discount Only card to a pharmacy within Caremark’s network.  A discount will be applied at the register.  Then, members can send receipts along with a completed HDHP Prescription Claim Form to the BEST Life’s Claims Department for processing.

Outlook Vision Discount Card
BEST Life provides an Outlook Vision Discount Card with our vision and HSA-compatible medical plans at no extra charge to you. This discount program offers access to over 10,500 optical centers and 10% to 50% off the regular retail price of eyeglasses, contact lenses, sunglasses, and corrective surgery. Members who have access to this discount program will receive an Outlook Vision Discount Card. QualSight Lasik Benefit
All BEST Life Dental, Medical, Vision and Life plans include a savings of 40 - 50% off the national average charge for QualSight procedures, which is administered by QualSight. BEST Life members are automatically eligible to receive discounts. Members should call 1-877-507-4448 before seeing any LASIK provider in order to obtain this discount.
