Group Dental plans
BEST Life’s PPO and indemnity dental plans are available for groups as small as 2 employees enrolling. Both products offer dental plans as employer-paid or voluntary, and employers can choose from a selection of national or regional PPO networks.
BEST Life PPO Dental
These dental plans have in-network incentives (i.e. higher in-network benefits) and give members the freedom to choose any dental provider of their choice or save on out-of-pocket costs with a network provider. Employers can choose from a variety of basic to rich plan design options. Child and adult orthodontia is also available.
This product is sold in AZ, CA, DC, FL, IL, IN, MD, MI, MO, NE, NV, OH, and PA .
A MAC PPO dental plan is available for Texas.
IndemnityPlus Dental
IndemnityPlus is BEST Life's indemnity plan that offers additional access to a PPO network. The same benefit levels apply in or out-of-network, and members can receive care from any US licensed dentist or specialist. Basic and rich plan designs are also available along with child and adult orthodontia.
Available in AK, AL, AR, AZ, CA, CO, DC, FL, GA, HI, ID, IL, IN, KS, KY, LA, MD, MI, MS, MO, MT, NC, ND, NE, NM, NV, OH, OK, OR, PA, SC, SD, TN, TX, UT, VA, WA and WY.
BEST Life Topaz Dental PPO & Indemnity Plans
These dental plans offer the right blend of choice and cost-savings features. Employers may choose from a set of indemnity or PPO plans.
PPO plans have in-network incentives (i.e. higher in-network benefits) and offer members greater cost-savings when services are provided by a network dentist. Our indemnity plans offer the same in and out-of-network benefits and give members the freedom to choose any dental provider of their choice. The option to access a PPO network for discounts is also available to Topaz Indemnity plan participants.

Health Plans
BEST Life offers two health plans for groups with 2 to 50 employees enrolling in select states:
Health Solutions II
For those who like traditional PPO health plans, our “Build Your Own” format gives you the choice to select the options you want:
- Broad range of deductible, co-pay, coinsurance and out-of-pocket maximum combinations;
- Preventive care benefits covered at 100%;
- CVS Caremark prescription drug coverage with the choice of 3-tier co-pay and deductible options or Discount only.
Health Solutions - High Deductible Health Plans
Our High Deductible Health Plan (HDHP) can be purchased alone or it can be paired with a Health Savings Account for significant tax savings. HDHP members can use any bank to open a Health Savings Account and can take advantage of tax benefits, ease of payment, and online account administration.
- A choice of deductible and coinsurance combinations;
- Preventive care benefits covered at 100%
- A discount only CVS Caremark prescription card included.

Employer-contributory Group Term Life for groups with 10 or more enrolling:
- Optional coverage includes Accidental Death & Dismemberment, Dependent and Supplemental Life benefits;
- Critical Illness, Cancer Care, Daycare and Repatriation of Remains available;
- Seat Belt, Airbag and Accelerated Death benefits included;
- Guarantee issue amounts are based on total group volume; and
- 2-year rate guarantee and discounts are available for preferred industries.
- Currently available in AK, AL, AR, AZ, CA, CO, DC, GA, HI, ID, IL, IN, KS, KY, MI, MS, NE, NM, NV, OH, OK, PA, SC, SD, TN, TX, UT and WY. Additional states pending approval.
Essential Group Life
A Group Term Life plan for groups with 2 or more employees enrolling
- Optional AD&D, Dependent and Supplemental Life
- Guarantee issue amounts are based on total group volume
- 2-year rate guarantee and discounts are available for preferred industries
- Essential Group Term Life is available in FL, LA, MD, MO, MT, NC, ND, OR, VA and WA.
- Voluntary policies available upon request.
BEST Life Silver
Offered as stand alone for groups with 10 or more employees enrolling, or for groups with 2 or more enrolled if purchased with another BEST Life plan.
- Guarantee issue based on age and group participation;
- Optional Accidental Death & Dismemberment (AD&D) benefit available for employees only;
- Seat Belt and Airbag benefit included with AD&D;
- Accelerated Death benefit included with basic coverage; and
- Supplemental Life available for employees and dependents who want to increase coverage.
- Currently available in AK, AL, AR, AZ, CA, CO, DC, GA, HI, ID, IL, IN, KS, KY, MI, MS, NE, NM, NV, OH, OK, PA, SC, SD, TN, TX, UT and WY. Additional states pending approval.
BEST Life Bronze
This Group Term Life plan is designed for employer-contributory groups with 2 to 9 employees enrolling.
- Optional Accidental Death & Dismemberment (AD&D) benefit available for employees only; and
- Supplemental Life available for employees and dependents.
- Currently available in AK, AL, AR, AZ, CA, CO, DC, GA, HI, ID, IL, IN, KS, KY, MI, MS, NE, NM, NV, OH, OK, PA, SC, SD, TN, TX, UT and WY. Additional states pending approval.

Short Term Disability
For employers who would like to provide a safety net for valued employees recovering from a non-work-related injury or illness. All plans require a minimum of 5 employees enrolling.
100% Employer-Paid
- No evidence of insurability required;
- No late entrants;
- Requires 100% participation;
- Percentage or flat benefit amounts up to $1,000 weekly for preferred industries;
- 13 or 26 weeks benefit duration;
- A choice of benefits for a sickness/injury to begin 1/8, 8/8, 15/15 or 30/30 days; and
- Partial Disability and Maternity coverage optional.
Employer-Contributory
- 25% minimum employer contribution;
- Requires 75% participation and a minimum of 5 employees enrolling;
- Percentage or flat benefit amounts up to $1,000 weekly for preferred industries;
- 13 or 26 weeks benefit duration;
- A choice of benefits for a sickness/injury to begin 1/8, 8/8, 15/15 or 30/30 days; and
- The option to add coverage for Partial Disability or Maternity.
Voluntary
- Requires 25% participation;
- Benefits paid at a percentage or a flat benefit up to $1,000 per week for preferred industries;
- Offers 13, 26 or 52 week benefit duration options;
- A choice of benefits for a sickness/injury to begin 1/8, 8/8, 15/15 or 30/30 days; and
- Coverage for maternity is optional.
Custom Plans
- May be offered as 100% employer-paid, employer-contributory or voluntary;
- Benefits paid at a percentage or a flat benefit up to $2,500 per week;
- May choose from 13, 26 or 52 week benefit duration periods;
- A choice of benefits for a sickness/injury to begin 1/8, 8/8, 15/15 or 30/30 days; and
- Maternity and/or Partial Disability benefits optional.
