Q. How does the plan work?
A.With BEST Life’s Stand Alone Vision and Access Vision plans there is no network; members and their dependents can go to any licensed ophthalmologist or optometrist for vision care. The member or their vision care provider will then file a vision claim form with BEST Life for reimbursement.
Q. How are benefits covered?
A. Your plan includes a yearly deductible, allowances and frequency. How you are reimbursed will depend on the combination of all of these:
- Yearly Deductible: the amount each member must meet before claims are paid.
- Allowances: the amount BEST Life will reimburse the member for that service. Each service has a different allowance amount.
- Frequency: The number of months exams, frames, lenses and contacts benefits becomes available for use again. Some plans may have different frequencies for each category of services.
Your plan’s Certificate Booklet provides detailed information about your plan. We encourage our members to become familiar with the Certificate Booklet before they use their benefits.
Q. Does my plan cover contacts in addition to frames and lenses?
A. Depending on your plan, contacts may be covered in lieu of frames and lenses, or may be covered in addition to frames and lenses.
If your plan covers contacts in lieu of frames and lenses, then you will be reimbursed for the contacts only. Frames and lenses will not be available for reimbursement until after the contacts benefit is ready for use again (based on the frequency for contacts).
If your plan covers contacts in addition to frames and lenses, then you can be reimbursed for all three items. Each item will still be subject to the plan’s frequencies.

Q. Does my plan include any discounts?
A.. Yes, your Vision plan includes discounts through one of the two Vision Discount Programs BEST Life offers.
Outlook Vision Discount Card
If your emplyer enrolled in the BEST Life vision plan before 2009, then your BEST Life coverage includes an Outlook Vision Discount Card. This discount program offers access to over 10,500 optical centers and 10% to 50% off the regular retail price of eyeglasses, contact lenses, sunglasses, and corrective surgery. To take advantage of these discounts, go to the Outlook website, www.outlookvision.com, to locate a vision care provider within the Outlook network. An Outlook Vision Discount Card will be required to obtain discounts.
EyeMed Vision Discounts
If your employer enrolled in 2009, you are eligible to receive discounts through EyeMed. Discounts for exams, glasses, lens options and add-ons, contact lens and even laser vision correction are included with no limitations on frequency. You can obtain these discounts through EyeMed’s network, which has access to over 21,000 locations nationally and include many leading optical retailers including LensCrafters, Target Optical, Sears Optical, JC Penny Optical and most Pearle Vision locations.
You will receive an explanation flyer and a Summary of Discounts along with a BEST Life ID card. A BEST Life ID card will be necessary to obtain discounts.
